Yes: They made it, it's the polite thing to do.
Sometimes: It depends on who made it, how I feel, and what it is.
Rarely: If I don't want it, I don't eat it.
Is your health being affected by your weight?
Yes: I have at least one weight / food related condition (diabetes, fatty liver, sleep issues, digestive issues, high blood pressure, etc...)
Maybe: I am not sure if I have issues related to my weight or food choices.
